Answer by Dr. Ru: About Abciximab usage in lactation

Monoclonal antibody fragment that binds to platelet receptors inhibiting platelet aggregation. Used to treat angioplasty and unstable angina. Since the last update we have not found any published data on its excretion in breast milk. Its pharmacokinetic data: very high molecular weight, high percentage of platelet binding (Harder 1999) and short half-life, make it very unlikely it will pass into breast milk in significant amounts. Due to its protein nature, it deteriorates in the gastrointestinal tract, not being absorbed. This low oral bioavailability would make it difficult for it to pass to the infant’s plasma ingesting breast milk, except in preterm infants and in the immediate neonatal period when there may be increased intestinal permeability.

Answer by DrLact: About Abciximab usage in lactation

No information is available on the clinical use of abciximab during breastfeeding. Because abciximab is a large protein molecule with a molecular weight of 47,615, the amount in milk is likely to be very low and absorption is unlikely because it is probably destroyed in the infant's gastrointestinal tract. Until more data become available, abciximab should be used with caution during breastfeeding, especially while nursing a newborn or preterm infant.
