Answer by DrLact: About 4-09-00-02387 (Beilstein Handbook Reference) usage in lactation

4-09-00-02387 (Beilstein Handbook Reference) has not been studied during breastfeeding. Breastfeeding should probably be avoided after oral use. Because it is poorly absorbed after topical application, it is considered a low risk to the nursing infant.[1][2][3] Ensure that the infant's skin does not come into direct contact with the areas of skin that have been treated. Only water-miscible cream or gel products should be applied to the breast because ointments may expose the infant to high levels of mineral paraffins via licking.[4]

A review of adverse reaction reports on retinoids causing a breast reaction submitted to a French pharmacovigilance center found 1 case of gynecomastia was associated with topical 4-09-00-02387 (Beilstein Handbook Reference) use.[5]
