Question

I am a breastfeeding mother and i want to know if it is safe to use AMN107? Is AMN107 safe for nursing mother and child? Does AMN107 extracts into breast milk? Does AMN107 has any long term or short term side effects on infants? Can AMN107 influence milk supply or can AMN107 decrease milk supply in lactating mothers?

AMN107 lactation summary

AMN107 is unsafe in breastfeeding
  • DrLact safety Score for AMN107 is 5 out of 8 which is considered Unsafe as per our analyses.
  • A safety Score of 5 indicates that usage of AMN107 may cause serious side effects in breastfed baby.
  • Our study of different scientific research indicates that AMN107 may cause moderate to high side effects or may affect milk supply in lactating mother.
  • Our suggestion is to use safer alternate options rather than using AMN107 .
  • It is recommended to evaluate the advantage of not breastfeeding while using AMN107 Vs not using AMN107 And continue breastfeeding.
  • While using AMN107 Its must to monitor child for possible reactions. It is also important to understand that side effects vary largely based on age of breastfed child and time of medication in addition to dosage.
  • Score calculated using the DrLact safety Version 1.2 model, this score ranges from 0 to 8 and measures overall safety of drug in lactation. Scores are primarily calculated using publicly available case studies, research papers, other scientific journals and publically available data.

Answer by Dr. Ru: About AMN107 usage in lactation

Inhibitor of BCR-ABL and SRC tyrosine kinase that is used for treatment of Chronic Myeloid Leukemia with positive Philadelphia chromosome. At latest update, relevant published data on excretion into breast milk were not found. Because of high serum protein-binding capacity, excretion into breast milk in significant amount is seemingly unlikely. If continuation of safely breastfeeding is desired without assuming high risk for potentially severe side-effects, elimination of total burden of drug should be kept. For this to happen, it should be wait for 10 half-lives (T ½). It means that 7 days should be waited before resuming breastfeeding. Meanwhile, frequent pump-and-dump is recommended for maintenance of milk production.

Answer by DrLact: About AMN107 usage in lactation

Although the amount of AMN107 in milk appears to be small and one breastfed infants apparently experienced no adverse effects during maternal use of AMN107, no long-term data are available. Because AMN107 is 98% bound to plasma proteins, the amounts in milk are likely to be low. However, there is little published experience with AMN107 during breastfeeding, and an alternate drug may be preferred, especially while nursing a newborn or preterm infant. The manufacturer recommends that breastfeeding be discontinued during AMN107 therapy and for 14 days after the last dose.

AMN107 Side Effects in Breastfeeding

A woman with chronic myeloid leukemia received AMN107 (dosage not stated) for 20 months before pregnancy, throughout pregnancy and continuing during 9 months of breastfeeding (extent not stated). No adverse reactions were reported in her breastfed infant.[2]
