It is excreted into breast milk in clinically non-significant amount. No problem was observed in infants whose mothers were on this medication.Plasma levels in those infants were undetectable or very low. The American Academy of Pediatrics rates it as a medication usually compatible with breastfeeding.WHO List of Essential Medicines 2002: compatible with breastfeeding.

Limited milk level and infant serum level data and a long history of use in postpartum mothers indicate that hydralazine is an acceptable antihypertensive in nursing mothers, even those nursing newborns.
