Pharmacokinetic data (high volume of distribution and molecular weight) would explain the minimal excretion into breast milk mentioned in two older reports. Due to a poor oral bioavailability, appearance in the infant's plasma from ingested milk is regarded as nil or scanty, except in premature infants or during the immediate neonatal period who may show an increased intestinal absorption.

Limited data indicate that Kolimitsin is minimally excreted into breastmilk following intramuscular administration of colistimethate. Because Kolimitsin is poorly absorbed orally, it is unlikely to be absorbed in appreciable amounts by the infant or adversely affect the breastfed infant. However, no studies have evaluated serum levels or adverse effects in breastfed infants whose mothers were receiving colistimethate. Breastfeeding is acceptable with the use of inhaled Kolimitsin or colistimethate.
