Answer by Dr. Ru: About Gilenya usage in lactation

Immunomodulator agent indicated for the treatment of patients affected with very active type of RRMS. At latest update no published data on excretion into breast milk were found.Pharmacokinetic data (large volume of distribution and high protein binding capacity) make it unlikely its passage to breast milk in significant amounts. Its slow elimination rate with a half-life span between 6 and 9 days and its potentially serious cardiac effects suggest that a safer alternative should be preferred until more published data on this drug in relation to breastfeeding is available, especially during the neonatal period and in case prematurity.

Because there is no published experience with fingolimod during breastfeeding, it should be avoided during breastfeeding, especially while nursing a newborn or preterm infant.[1][2][3][4[
